Folate Receptor Antibody Test (FRAT)
At Brainchild Neurology, we use diagnostic testing thoughtfully and only when it is appropriate for a child’s specific situation. One test that families sometimes ask about is Folate Receptor Alpha Autoantibody (FRAT) testing.
What is FRAT testing?
FRAT testing is a blood test that looks for antibodies to the folate receptor alpha, which plays a role in how folate is transported into the brain. In certain cases, the presence of these antibodies has been associated with cerebral folate deficiency and some neurodevelopmental conditions.
Research in this area is ongoing, and FRAT testing is not a routine test. It does not provide a diagnosis on its own, but in some situations, it may offer additional information that helps guide care.
When is FRAT testing considered?
FRAT testing may be considered when requested by families and when clinically appropriate, based on a child’s history, symptoms, and prior evaluations. It is not recommended for every child and is not ordered automatically.
How can we schedule FRAT testing?
Any family seeking FRAT testing for their child will need to first schedule a New Patient (intake) visit. At that appointment we can explore whether or not FRAT is appropriate for your child. Let us know when you are scheduling your initial visit that you are interested in FRAT for your child.
We take time to discuss:
what the test can and cannot tell us
whether it is likely to be helpful for the child
how results would (or would not) change management
In order to run a FRAT test, we coordinate ordering the laboratory kit and arranging a blood draw with a phlebotomist. We will schedule a follow up appointment shortly after the results are available so that we can review results and discuss next steps.
We believe testing should be paired with clear explanation and continuity of care, not just a lab report.

Follow up appointment - $275 (Our standard follow up rate)
*Our team coordinates the entire blood draw process, which can be difficult to arrange for this specialized test. We schedule an experienced phlebotomist to meet you at our office (sometimes at-home phlebotomy is an option depending on location and other factors), ensure proper handling of the kit, and manage all logistics so the process is seamless for your family.
